Filing A
Claim
(If you have a medical
emergency, skip steps 1 & 2, and go to the nearest
hospital).
In the event of injury or
illness, the Insured should :
1. Visit the campus
Student Health Center
2. If the Student Health
Service is not available or further treatment is
required, contact NHBC (PPO Network) at :
1-888-621-7900,
PIN # AMA411
for a referral to a network doctor.
3. Present your insurance
ID card to the doctor and follow doctor's
instructions.
4. Complete a Claim Form
and attach all itemized bills, statements and
receipts and mail to the Claims Division.
